The basics

Four things decide what you pay

  • The formulary

    Each plan covers a specific list of drugs. If yours isn't on it, you pay full retail or file for an exception.
  • The tier

    Covered drugs sit on tiers from preferred generic to specialty. Two plans can cover the same drug at very different tiers.
  • The pharmacy

    Preferred pharmacies cost less than standard ones. Filling the same prescription on Jefferson Boulevard versus Greenville Avenue can change your copay.

    Include dosage and how often you fill it. Bring the bottles or a printout from your pharmacy, guessing is where errors start.

    Confirm your pharmacy

    Tell us whether you fill along Skillman Street, near Forest Lane, or elsewhere in Dallas, and whether mail order works for you. Preferred status matters as much as the plan itself.

    Compare total annual cost

    Premiums plus deductible plus every expected copay across the year, never the monthly premium alone.

    Recheck each fall

    Formularies and tiers reset January 1. A plan that was perfect last year can be the wrong one this year.

Watch out

Penalties, gaps and help paying

If you go 63 days or more without creditable drug coverage after you're first eligible, Medicare adds a late enrollment penalty to your Part D premium, permanently. It's small each month and large over a retirement, and it's one of the mistakes we see most often among new Dallas enrollees.

Lower-income Dallas County residents may qualify for Extra Help, which reduces or eliminates Part D premiums and deductibles. Local aging services can walk you through the application; see our Dallas senior resources page.

Employer or VA coverage may count

Creditable coverage from a job, TRICARE or the VA usually means no penalty. Hold on to the annual creditable-coverage letter your plan sends you.

Advantage plans usually include Part D

If you're on a Medicare Advantage plan with drug coverage, don't add a standalone Part D plan on top, it can drop you out of your Advantage plan entirely.

Medicare now lets you spread out-of-pocket drug costs across monthly payments instead of paying a large amount at the pharmacy counter early in the year. It doesn't lower your total cost, it smooths it out. Worth asking about if a January refill has ever hurt. Enrollment goes through your drug plan, and we'll walk you through it.

Your annual medication review

Between October 15 and December 7, every Part D plan can change its premium, deductible, formulary, tier placement and preferred pharmacy network. Your prescriptions change too. A plan that was the cheapest option last January can quietly become one of the most expensive by fall.

Common questions

Part D questions we hear in Dallas

Why is the plan with the lowest premium not the cheapest for me?
Because premium is roughly a tenth of the story. Deductible size, tier placement and your pharmacy's network status usually outweigh a few dollars a month in premium by a wide margin.
What if my drug isn't on the formulary?
You can ask your doctor to request a formulary exception, switch to a covered alternative, or wait for the next enrollment window to pick a plan whose formulary already includes it.
What is step therapy?
The plan requires you to try a lower-cost drug first before it will cover the one your doctor prescribed. Ask about step therapy rules before enrolling if you're on a brand-name medication.
Is mail order always cheaper?
Often for 90-day maintenance drugs, not always. We compare your Dallas pharmacy against mail order pricing rather than assuming one is automatically better.
Can I have Part D with a Medigap plan?
